Key Responsibilities\n

• Establish trusting relationships with patients and families by serving as a primary point of contact for community resources, assisting in aftercare arrangements tailored to individual needs.

• Identify and effectively manage resistance and conflict, working collaboratively with clients, their families, healthcare providers, and community representatives to foster resolution and understanding.

• Maintain comprehensive and current documentation of patient assessments, intervention plans, family involvement, and communications using established protocols and systems.

• Utilize your understanding of the DSM diagnostic criteria to identify relevant behavior patterns in clients, providing insights into their emotional and psychological needs, especially concerning age-related developmental tasks.

• Implement evidence-based strategies such as behavior modification techniques and assertiveness training as applicable in supportive therapy sessions.

Patient Population / Client Focus\n

In this position, you will principally work with terminally ill patients and their families, navigating the emotional complexities of grief, loss, and care transitions. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory.

• You must hold an active California license as a Clinical Social Worker (LCSW).
