Key Responsibilities\n

Your day-to-day responsibilities will include:

• Providing family-centered clinical behavioral health services, in alignment with CHDC's commitment to enhancing quality standards and patient health outcomes.

• Assisting patients with mental health concerns in navigating both the physical and behavioral healthcare systems, ensuring a seamless experience in integrated care.
\n
• Educating patients and their families about available resources and services that support wellness, disease prevention, and an overall healthy lifestyle.
\n
• Proactively participating in multi-disciplinary team meetings to discuss patient care strategies and contribute to comprehensive treatment plans.
\n
• Timely reporting of crisis situations and unusual incidents to the Medical Director, with immediate escalation of psychiatric and medical emergencies to the Chief Medical Officer.
\n
• Maintaining detailed and accurate documentation of patient interactions and treatment strategies in clinical records, ensuring compliance with healthcare regulations.
\n
• Engaging in Quality Improvement programs and contributing insights to elevate care quality; participation in all assigned training and meetings is essential.
\n
• Utilizing evidence-based screening tools to assess and report on individual healthcare needs.
\n
• Collaborating with patients suffering from Opioid Use Disorder as part of our specialized Center of Excellence Team.

Patient Population / Client Focus\n

This role predominantly serves hospice patients, focusing on those dealing with terminal illnesses and their families. Your expertise in clinical social work will be instrumental in helping these individuals navigate the complexities of end-of-life care while providing emotional and mental support to enhance their quality of life during this difficult time.

Qualifications\n

To be successful in this role, candidates must possess:

• A master’s degree in social work with an active Licensed Clinical Social Worker (LCSW) certification.

• A minimum of 2-3 years of relevant professional experience in social work, particularly within healthcare settings.

• Strong collaborative skills to effectively work with diverse healthcare professionals, social service providers, and patients.

• The ability to travel to various CHDC sites for meetings, outreach functions, and training opportunities as required.

Preferred Experience\n

Preference will be given to candidates with experience in hospice care settings or palliative care environments. Familiarity with the unique challenges faced by patients nearing end-of-life and their families will be advantageous.

We offer an attractive compensation package, with a salary between USD 90,000 and 100,000 annually.
