Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand patients' emotional and behavioral challenges.

• Diagnose and treat a variety of mental health disorders, adjusting care plans based on ongoing evaluations of patient progress.

• Collaborate with a multidisciplinary team, including healthcare providers and community resources, to develop effective discharge plans and ensure continuity of care post-therapy.

• Act as both an advocate and a coordinator for patients and their families to facilitate communication among all parties involved in their care.

• Monitor and manage patients at risk for potentially avoidable delays or readmissions; initiate preventive interventions to enhance patient outcomes.

• Participate in regular consultations with colleagues, offering expertise in troubleshooting complex case scenarios that may arise.

• Educate patients and their family members about available mental health resources, treatment options, and community support services.

Qualifications\n

• A Master's Degree in Social Work or a related field, with education obtained from an accredited institution.
