Work Environment

This position allows for the flexibility of working remotely while being part of a dynamic team within a community health clinic setting. Your typical work schedule will be Monday to Friday from 8:00 AM to 5:00 PM, with occasional evening shifts required based on patient needs. Key Responsibilities

• Conduct thorough mental health assessments that consider how medical conditions influence patients' mental health status.
• Develop and implement integrated treatment plans that address both psychiatric and medical needs, ensuring a cohesive care experience.
• Facilitate individual and group therapy sessions utilizing evidence-based approaches tailored to the unique circumstances of each patient.
• Collaborate with medical providers to ensure a coordinated treatment plan and optimize patient outcomes.
• Provide crisis intervention and support to patients during mental health emergencies, ensuring immediate and effective care.
• Educate patients about the relationship between mental and physical health, empowering them to manage their conditions proactively.
• Maintain accurate, confidential patient records that conform to legal and ethical standards.
• Participate actively in team meetings and contribute to case conferences and interdisciplinary care planning efforts.
• Stay updated on the latest research and best practices in integrated mental health care.

Patient Population / Client Focus

In your role, you will primarily serve a diverse patient population, including individuals with co-occurring mental health and medical conditions such as chronic pain, autoimmune disorders, and substance use disorders. By understanding the complexities of each patient's unique circumstances, you will deliver tailored therapeutic interventions that enhance their overall well-being.

Qualifications

• Education: A Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.
• Licensure: Candidates must possess current and valid licensure as an LICSW, LMHC, or LMFT in Washington State.
• Skills and Expertise: Proficiency in addressing mental health needs of individuals with co-occurring medical conditions is essential.
• Collaboration: Experience delivering integrated care and working closely with medical professionals is highly valued.
• Communication: Excellent interpersonal and communication skills are crucial for successful patient interactions and team collaboration.
• Technical Proficiency: Familiarity with electronic health record (EHR) systems and various computer applications is desirable.

Preferred Experience

• A minimum of 2 years of demonstrated experience in mental health counseling, particularly with patients facing both psychiatric and medical challenges.

Compensation and Benefits
