Key Responsibilities\n

• Engage with our interdisciplinary care team to ensure comprehensive communication, effectively facilitating patient referrals and the development of integrated treatment plans.

• Monitor and evaluate each client’s treatment plan, making necessary adjustments, extending, or closing plans as required for optimal patient outcomes.
\n
• Conduct telephonic evaluations and screenings and promptly document findings to maintain accurate patient records.
\n
• Collaborate closely with the care team to achieve specific goals for patient outcomes, creating a cohesive approach for each client.

• Design and coordinate treatment strategies tailored to each patient’s health service needs, effectively integrating community resources where applicable.

• Utilize our electronic medical record (EMR) system to complete documentation in compliance with regulatory standards and clinical guidelines.

Patient Population / Client Focus\n

In this role, you will cater primarily to a diverse population of families and individuals facing various psychological challenges, including anxiety, depression, trauma, and relationship issues. Your therapeutic interventions will be crucial in addressing mental health needs and fostering resilience within the community.

Qualifications\n

• A Master’s Degree in Marriage and Family Therapy or a related field.

• Active licensure as a Licensed Marriage and Family Therapist (LMFT) in California.

• A minimum of three years of experience in an outpatient setting, demonstrating a robust understanding of clinical practices.

• Strong computer skills, including proficiency with word processing, email systems, spreadsheets, and electronic medical records.

• Excellent time management capabilities and strong organizational skills to handle multiple caseloads effectively.

Preferred Experience\n

Candidates with experience in telehealth settings will be particularly appreciated, as will those with a background in culturally competent care practices. Familiarity with evidence-based therapy modalities is a plus.

Compensation and Benefits\n
