Key Responsibilities\n

• Conduct thorough and comprehensive assessments to identify the social, emotional, and environmental challenges faced by clients.

• Provide individual and family counseling to clients confronting various life difficulties, employing a trauma-informed care approach.

• Utilize crisis intervention techniques, perform risk assessments, and create comprehensive safety plans as necessary.

• Collaborate with community organizations to coordinate services effectively, advocating for clients' best interests and ensuring seamless access to resources.

Patient Population / Client Focus\n

You will work primarily with a diverse population of individuals facing terminal illnesses, their families, and caregivers in a hospice setting. Your support will be crucial in navigating their emotional and practical needs, facilitating discussions around end-of-life care, grief, and bereavement, while also connecting them to vital community resources. With a strong emphasis on building relationships, you will empower your clients to express their needs and make informed decisions during a pivotal time in their lives.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
