Work Environment\n

This position is predominantly a remote role, allowing you the flexibility to manage your workspace while remaining engaged with our clients. You will be working from home with opportunities to connect through telehealth sessions. The job may occasionally require some onsite presence at our Hospice Center, particularly for critical assessments or team meetings, fostering a strong connection with your colleagues and clients.

Key Responsibilities\n

• Conduct thorough, impartial psychosocial assessments for patients, utilizing your education, training, and clinical experience to inform your evaluations.

• Develop individualized treatment plans in collaboration with clients, ensuring that each plan is tailored to meet unique psychological and social needs.

• Implement evidence-based therapeutic interventions, including solution-focused therapy and motivational interviewing techniques, adapting your approach based on ongoing assessments of patient progress.

• Work alongside a multidisciplinary team that includes primary care practitioners and other healthcare providers to provide comprehensive, integrated care that addresses both physical and mental health needs.

• Regularly monitor client responses to therapy, using established metrics to assess progress towards treatment goals and adjusting strategies as necessary.

• Assist medical professionals in the oversight of psychotropic medications, focusing on adherence, side effects, and overall effectiveness to ensure optimal treatment outcomes.

• Prepare and maintain accurate patient records, documenting assessments, treatment plans, and progress notes in accordance with state licensure regulations and clinic policies.

Patient Population / Client Focus\n

This role primarily involves working with adults experiencing a range of mental health issues, including anxiety, depression, grief, and chronic illness. You will be instrumental in helping clients navigate their mental health challenges through supportive counseling and therapy, tailoring your approach based on their individual circumstances and treatment goals.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in the State of New York.

• A minimum of 1 year of experience providing therapy as an independently licensed social worker, preferably in a mental health setting.

Compensation and Benefits\n
