What You'll Do

Provide high-quality, patient-centered physical therapy services in patients' homes.

Evaluate patients' mobility, strength, functional abilities, and rehabilitation needs.

Develop and implement individualized treatment plans focused on improving independence and quality of life.

Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.

Educate patients and families on exercises, therapy techniques, safety, and lifestyle modifications.

Collaborate with nurses, physicians, therapists, and other healthcare professionals to provide coordinated care.

Maintain accurate and timely documentation of patient visits and progress.
