We pride ourselves on a supportive team culture that promotes effective communication, shared learning, and collaborative care practices.Key ResponsibilitiesConduct thorough evaluations of patients, including reviewing medical histories, performing specific assessments, and making evidence-based recommendations for treatment plans.Engage with patients and their families by clearly explaining evaluation findings and collaboratively setting realistic goals for therapy outcomes.Implement personalized treatment protocols tailored to individual patient needs while adapting to the evolving healthcare landscape.Monitor and document changes in patient conditions, making necessary adjustments to the plan of care and consulting with other healthcare professionals as appropriate.Participate in comprehensive discharge planning and follow-up care, ensuring accurate and timely documentation of all patient interactions and treatment results.Manage your time effectively to meet productivity expectations while maintaining an accurate schedule and attending all necessary meetings.Contribute to the achievement of departmental goals and strategies, ensuring compliance with health regulations and accreditation standards.Proactively pursue continuing education opportunities, enhancing your clinical skills and knowledge while mentoring and supervising students as needed.Facilitate exceptional customer experiences by being flexible and responsive to patient needs, actively contributing to initiatives aimed at improving access to care.Patient Population / Client FocusIn this role, you will work with a diverse group of patients who require physical therapy services for various conditions, including post-surgical recovery, chronic pain management, and rehabilitation from injuries. Your expertise will be crucial in tailoring treatment approaches that cater to the unique circumstances and challenges faced by patients in their home environments.QualificationsA Doctorate degree in Physical Therapy (DPT) is required.Must possess a valid DPT license in the State of New Jersey, along with any additional state certifications as applicable.Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandatory.While a minimum of two years of experience is preferred, specific prior work experience in home care settings is not required; candidates with a strong desire to learn and grow in a supportive environment are encouraged to apply.Preferred ExperienceIdeal candidates will have hands-on experience in outpatient therapy settings, familiarity with electronic medical record (EMR) systems, and a proven track record of implementing innovative treatment modalities.
