Advanced Practice Provider (APP) - Nurse Practitioner (Travel)

Position Title Advanced Practice Provider (APP) – Nurse Practitioner (Travel) Job Summary The Advanced Practice Provider coordinates the continuum of care for patients referred to the University of South Carolina Brain Health Network (UBHN). The role involves communicating with primary care providers, arranging ancillary services, and ensuring timely services and communication. Travel across South Carolina is required. Key Responsibilities Primary point of contact for all primary care patient referrals within the UBHN, taking calls from providers/offices and connecting with patients. Ensure all clinical services for referred patients are received timely and appropriately, and results are communicated within the UBHN following established protocols. Follow up with patients to provide appointment reminders and ensure patient comprehension and compliance. Collaborate with assigned physician(s) to discuss care and management of assigned patients. Accept and perform other related duties as assigned. Minimum Qualifications Bachelor’s degree in a related field and at least 3 years of job-related experience, or an equivalent combination of certification, training, education, and/or experience. Current licensure as a Nurse Practitioner in South Carolina with LLR, in good standing. Preferred Qualifications At least one year of experience working in a physician practice. Experience coordinating care of patients across different areas/services of a health system. Knowledge, Skills, and Abilities Excellent written and verbal communication skills. Ability to assess, identify, and address individual patient needs. Knowledge of medical terminology, use of medical equipment, and ability to follow clinical orders.
