Advanced Practice Provider - Nurse Practitioner The Advanced Practice Provider (APP) plays a pivotal role in coordinating the continuum of care and managing the overall patient experience within the USC Brain Health Network (UBHN). The APP ensures effective communication, efficient operations, and patient-centered service delivery for all patients referred to UBHN between primary care providers and specialized services. Job Summary Coordinate and navigate patient care across the UBHN, ensuring timely and appropriate clinical services and effective communication between primary and specialty providers. Job Related Minimum Required Education and Experience Bachelor’s degree in a job-related field and at least 3 years of related experience, or an equivalent combination of certification, training, education, and/or experience. Required Certification, Licensure & Other Credentials Current licensure as a Nurse Practitioner with SC LLR, in good standing. Preferred Qualifications At least one year of experience working in a physician practice. Experience coordinating care across different areas or services of a health system. Knowledge / Skills / Abilities Effective communication. Ability to assess, identify, and address individual patient needs. Knowledge of or ability to learn medical terminology, use medical equipment, and follow clinical orders. Job Duties Serve as the primary point of contact for all primary-care patient referrals within UBHN, taking calls from providers and connecting with patients. Ensure referred patients receive all clinical services promptly and that results are communicated within UBHN per established protocols. Follow up with patients to provide appointment reminders and ensure comprehension and compliance. Collaborate with assigned physicians to discuss care and management of assigned patients regularly.
