Street Medicine Peer Navigator Housing & Care Coach
Healthcare in Action, Inc
Healthcare in Action, Inc. is seeking a Peer Navigator to support members experiencing homelessness in Hollywood, West Hollywood and Koreatown CA. This full-time field role involves building trust, coaching on health behaviour changes, and coordinating care with ECM/CSS under CalAIM.
You will work 8-hour shifts Monday–Friday, with up to 50% travel, driving the mobile van and logging data in HMIS systems. Strong communication, reliability, and housing/resource navigation skills are essential.
