Key Responsibilities\n

• Conduct comprehensive assessments and evaluations of clients to identify their mental health needs, relevant social determinants, and create tailored treatment goals that guide their therapeutic journey.

• Facilitate one-on-one counseling sessions, addressing a range of mental health issues, including anxiety, depression, trauma, and family dynamics, employing evidence-based therapeutic modalities.

• Lead group therapy sessions that promote discussion on common mental health concerns, build support networks, and offer psychoeducation to clients facing similar challenges.

• Develop, implement, and routinely review personalized treatment plans, ensuring that they incorporate measurable goals and effective interventions tailored to each client's unique situation.

• Provide critical crisis intervention services, including conducting risk assessments, safety planning, and coordinating emergency resources for clients in acute distress.

• Advocate for clients’ rights and facilitate their access to necessary services, guiding them through the complexities of healthcare and community resources available within Jersey City.

• Maintain accurate and confidential client records in compliance with legal and ethical standards, encompassing assessments, treatment plans, progress notes, and client outcomes.

Patient Population / Client Focus\n

The social worker will primarily work with individuals and families facing mental health challenges within the hospice center environment and broader community. This includes providing support to clients dealing with grief, loss, and end-of-life issues, necessitating a client-centered, strength-based approach that respects the complexities of their life situations and emotional states.

Qualifications\n

• A Bachelor’s degree in Social Work is required. While a Master’s in Social Work (MSW) from an accredited institution is preferred, applicants with prior LCSW experience will be highly considered.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this role.

• Clinical experience in social work or mental health counseling, particularly in a remote setting, is highly desirable.

Compensation and Benefits\n

The compensation for this role ranges from USD 80,000 to 95,000 annually, commensurate with experience.
