Key Responsibilities\n

• Conduct thorough ongoing assessments to identify the specific needs of each client and to tailor therapeutic interventions effectively.

• Facilitate communication among supervisors, direct care staff, family members, and other key stakeholders to keep everyone informed about clients’ progress.

• Develop and manage schedules for appointments, home visits, and progress monitoring to ensure continuity of care and therapeutic success.

• Coordinate home visits for comprehensive assessments, including in-home observations and chart reviews, to gain a full understanding of client dynamics.
\n
• Ensure the effective implementation of individualized program plans, regularly evaluating their progress and making necessary adjustments.
\n
• Participate actively in interdisciplinary team meetings and relevant case discussions to collaboratively enhance treatment approaches.
\n
• Document and complete detailed progress notes on a monthly and quarterly basis to track improvements and guide future treatment decisions.

• Organize and implement training sessions to ensure that all staff members are competent in program plan implementation and documentation protocols.

• Provide follow-up interviews and ongoing support through home visits, ensuring that clients receive the assistance they need as challenges arise.

• Maintain accurate records that are easily accessible to relevant parties, ensuring compliance with all legal and ethical standards.

• Serve as a liaison to the public and the mental health community, helping to foster partnerships that support the organization’s development goals.

Qualifications\n
