Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of clients and their families to identify needs and facilitate access to necessary services.

• Engage actively in the formulation and implementation of the Individualized Family Service Plan (IFSP) in collaboration with interdisciplinary teams, ensuring holistic support for each family.

• Monitor and assess outcomes for clients, adjusting care plans collaboratively to reflect changes in needs or circumstances.
\n
• Provide emotional support and guidance to families coping with the terminal illness of a loved one, including facilitating important conversations concerning goals of care and advance planning.

• Adhere to all relevant federal and state regulations while ensuring the health and safety of both clients and staff.
\n
• Utilize evidence-based practices when developing joint plans with families to guide service delivery and recommend appropriate interventions.

• Enhance family engagement through coaching models and natural environment teaching techniques tailored to individual needs.

• Provide educational resources and support to families, helping them gain skills to promote their loved ones’ emotional and psychosocial well-being.

Patient Population / Client Focus\n

You will primarily serve families in hospice care settings, focusing on those experiencing the unique challenges presented by terminal illness. Your role will emphasize helping these families navigate complex feelings, making emotionally charged decisions, and ensuring that their loved ones receive compassionate and personalized end-of-life care.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited program.

• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.

• A minimum of one year of relevant experience working with families in hospice and end-of-life care settings.

Compensation and Benefits\n

Norwood Counseling Center offers a competitive salary ranging from USD 75,000 to 80,000 annually, commensurate with experience.
