Key Responsibilities\n

• Collaborate closely with our interdisciplinary healthcare team to create and implement individualized treatment and discharge plans based on comprehensive psychosocial assessments and established social work interventions.

• Forge and maintain strong relationships with community resource groups and organizations, representing Redesign Counseling Hub at meetings and on committees focused on addressing the social needs of our patients.

• Provide crisis intervention and counseling services to clients, effectively assessing and treating emotional and behavioral disorders through established therapeutic frameworks.

• Communicate regularly with physicians and other healthcare professionals to report on patient progress, address concerns and needs, and identify emerging challenges to care.

• Ensure meticulous documentation of client progress and treatment plans to uphold clinical standards and comply with department policies.
