Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs to diagnose mental health conditions and formulate personalized treatment plans.

• Provide case management services, which may include crisis interventions and short-term counseling, tailored to each client's immediate needs.

• Assist clients in navigating healthcare systems by guiding them in completing entitlements, accessing supplemental insurance, and securing other benefits aimed at reducing dependence on emergency aid.

• Facilitate access to prescription cost-assistance resources through local community programs and pharmaceutical initiatives.
\n
• Advocate for clients’ needs while coordinating with community agencies and resources, ensuring they receive comprehensive support.

• Maintain diligent records of patient interactions, treatment plans, and outcomes, preparing necessary reports and documentation as required.

• Plan and coordinate transportation assistance for patients when they must attend essential in-person appointments.

• Participate in clinical meetings and team discussions to review patient progress and refine treatment approaches, enhancing collaborative care.

Qualifications\n

Compensation and Benefits\n
