Key Responsibilities\n

• Assist primary care providers in identifying and managing mental health and psychosocial issues, contributing to a cohesive care team that prioritizes patient outcomes.

• Conduct thorough patient intakes, assessing diagnostic and functional needs, and providing actionable treatment recommendations to primary care providers.

• Deliver focused, brief interventions to patients requiring mental health support, ensuring a swift response to their needs.

• Evaluate patient progress through regular follow-ups, making adjustments to treatment plans as necessary to optimize therapeutic outcomes.

• Develop personalized relapse prevention strategies for patients striving to maintain stability in their mental health.
\n
• Identify 'at risk' patients and collaborate on preventive care plans to mitigate the deterioration of their mental health.
\n
• Coordinate health service delivery related to behavioral health, maintaining effective communication with all treating professionals, both within and external to the primary care setting.
\n
• Document patient progress meticulously within treatment charts while keeping primary care providers informed about ongoing care and progress.
\n
• Support patients in adhering to medical treatments initiated by primary care providers, offering guidance on coping with any medication side effects.
\n
• Engage in consultations with clinic management and team members to evaluate program effectiveness and suggest behavioral service improvements.

Qualifications\n

• Master’s Degree in Clinical Counseling or a closely related field.

• Knowledge of DSM-IV diagnoses, psychotherapeutic theories, and techniques.

• Ability to assess individual patient needs and deliver effective treatment plans.

• Excellent interpersonal and communication skills to foster the relationship between patients and the healthcare team.

• Ability to work collaboratively in an interdisciplinary environment.

Preferred Experience\n

• Experience in conducting family therapy, group therapy, and child psychotherapy is highly desirable.

• Previous work within a Federally Qualified Health Center (FQHC) setting is an advantage.

• Bi-lingual proficiency in English and Spanish is preferred but not mandatory.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 – 105,000 per year, commensurate with experience and qualifications.
