Key Responsibilities\n

• Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, ensuring a thorough understanding of their unique needs.

• Develop and implement individualized service plans, encompassing clear goals, targeted interventions, and ongoing progress monitoring.

• Provide direct counseling and emotional support to individuals and families facing a broad spectrum of life challenges, including mental health issues and access to community services.

• Engage clients actively in the development of personalized service plans, thereby fostering their autonomy and facilitating connections to essential community resources.

• Provide immediate crisis intervention, carry out risk assessments, and develop safety plans for clients in urgent situations.

• Collaborate with community organizations and other service providers to ensure a coordinated approach to client care and advocacy.

• Participate in community outreach initiatives to elevate awareness of agency offerings and promote access to vital social services.

• Actively contribute to case conferences and staff meetings, sharing insights and learning collaboratively with colleagues.

Patient Population / Client Focus\n

This role focuses on serving diverse populations in the Mesa community, prioritizing those who are marginalized or underserved. Clients may include individuals experiencing homelessness, victims of domestic violence, individuals battling substance abuse issues, and families living in poverty. Understanding the social determinants of health and employing trauma-informed care practices will be essential in effectively supporting our clients.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.

Compensation and Benefits\n
