Responsibilities Conduct MDS assessments for residents in a timely and accurate manner, in compliance with federal regulations and company policies Collaborate with the healthcare team to ensure accurate and complete data collection, including reviewing medical records and obtaining necessary information from healthcare providers Develop and maintain complex care plans, incorporating resident goals and preferences Coordinate with care teams to ensure seamless transitions and effective ongoing care Participate in Quality Improvement initiatives to enhance resident outcomes and reduce risk Develop and maintain strong relationships with residents, families, and staff to ensure exceptional customer service About the Role As an MDS Coordinator with Wedgemere Health Care, you will be working in a dynamic and supportive environment that values teamwork, innovation, and resident-centered care.
