Key Responsibilities

• Conduct thorough patient evaluations, taking comprehensive medical histories and utilizing specific assessments to formulate effective treatment plans tailored to individual patients' needs.
• Clearly communicate assessment findings and treatment strategies to patients and their families, actively involving them in the goal-setting process to ensure alignment with their rehabilitation objectives.
• Demonstrate proficient application of treatment protocols, remaining adaptable to the evolving healthcare landscape and the dynamic needs of your patients.
• Consult with various healthcare professionals as necessary, recognizing and documenting any changes in a patient's condition that may necessitate modifications to their care plan.
• Engage in effective discharge planning and follow-up care, meticulously documenting all patient care activities, including evaluations, progress notes, and discharge summaries to create a comprehensive medical record.
• Manage time effectively and meet productivity expectations while maintaining an organized schedule and completing daily tasks punctually.
• Participate in community outreach and marketing activities, helping to elevate the visibility and reputation of our facility.
• Contribute actively to the establishment and attainment of departmental goals, ensuring compliance with all relevant regulations and accreditation standards.
• Pursue ongoing professional education and training opportunities, serving as a mentor to students and new practitioners within the organization.

Patient Population / Client Focus

Our Rehabilitation Center serves a diverse patient population, ranging from post-operative rehabilitation cases to individuals with chronic pain or mobility issues. You will work with patients of various ages and backgrounds, tailoring your approach to fit their unique conditions and goals. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy, ensuring compliance with regional regulatory standards.

• Proficient in utilizing modern word processing and other computer skills relevant to patient documentation and care management.

Preferred Experience

While a minimum of two years of professional experience is preferred, new graduates with a strong clinical internship background are encouraged to apply. A background in orthopedic and neurological rehabilitation will be beneficial.
