Key Responsibilities\n

• Deliver timely clinical behavioral health services to clients, ensuring alignment with CHDC’s mission and quality standards.

• Assist mental health consumers in navigating both physical and behavioral healthcare systems, fostering an integrated approach to their health and wellbeing.

• Educate individuals on available resources, encouraging participation in programs that promote disease prevention, wellness, and an overall healthy lifestyle.

• Participate actively in multi-disciplinary treatment team meetings to develop coordinated care plans for patients.

• Report any crises or unusual incidents to the Medical Director promptly; escalate psychiatric and medical emergencies immediately to the Chief Medical Officer.

• Maintain comprehensive and accurate documentation of patient interactions and clinical assessments in electronic health records, as dictated by organizational protocols.

• Engage in quality improvement initiatives as assigned, contributing to a continuous learning environment through training sessions and collaborative meetings.

• Uphold all organizational policies and procedures, ensuring compliance to guarantee the integrity of our health center's operations.

• Utilize evidence-based screening tools to assess patients’ healthcare needs, providing necessary reports and actionable insights for treatment planning.

• Work closely with patients dealing with Opioid Use Disorder, collaborating with our specialized Center of Excellence Team to deliver coordinated care and recovery support.

You will primarily support individuals facing mental health challenges, including those with substance use disorders within our detox environment. Your work will directly impact the recovery journeys of patients by providing emotional support, education, and access to integrated care services. Qualifications\n

• Master's degree in social work from an accredited institution.
