This position is designed to be performed remotely, allowing flexibility while remaining connected with colleagues and patients. You will engage in a collaborative virtual environment, utilizing modern communication technologies to facilitate patient care and participate in team discussions. While patient interactions may be virtual, you may occasionally need to visit local healthcare facilities or community partners to enhance patient support and resource engagement.

Key Responsibilities\n

• Conduct thorough assessments and evaluations of patients to determine their mental health status, utilizing recognized tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) to inform treatment planning.

• Develop and implement individualized care plans aimed at addressing specific patient needs, goals, and desired outcomes, all while being mindful of cultural diversity and personal circumstances.

• Monitor patient progress regularly and adjust treatment strategies as necessary, focusing on proactive interventions that promote well-being and symptom management.

• Maintain accurate and timely clinical documentation and treatment records in accordance with regulatory guidelines, utilizing electronic health record (EHR) systems effectively.

• Collaborate with patients, families, and other healthcare providers to foster a cohesive and supportive care environment, ensuring all voices are included in the treatment process.

• Engage in the coordination of referrals to additional healthcare services and maintain relationships with external clinicians and agencies to facilitate comprehensive care.

• Provide educational resources and mentoring for students, interns, and other team members to enhance their learning experiences within the clinical setting.

• Participate in peer review, clinical collaboration, and chart audits under the guidance of your clinical supervisor to facilitate team development and improve service delivery.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Massachusetts.

Compensation and Benefits\n
