Work Environment

This position is based onsite in Fort Worth, Texas, at our specialized home care facility, which allows us to reach patients in their own living environments and provide personalized care. Working on-site offers direct engagement with both patients and our multidisciplinary team, enhancing collaboration and continuity of care. Regular business hours are from 9 am to 5 pm, ensuring a balanced work schedule conducive to personal well-being.

Key Responsibilities

• Conduct comprehensive psychiatric assessments and evaluations to accurately diagnose a variety of mental health conditions.
• Administer medication management plans, carefully monitoring the effectiveness of prescribed treatments and making adjustments as necessary.
• Collaborate closely with primary care providers, therapists, and community organizations to create and implement integrated care plans tailored to each patient's needs.
• Provide crisis intervention support, delivering immediate assistance during times of emotional distress or psychiatric emergencies.
• Engage in community outreach initiatives, promoting mental health awareness through education and workshops aimed at reducing stigma.
• Deliver therapeutic support and psychoeducation through counseling sessions, empowering patients with coping strategies and resilience-building skills.
• Maintain accurate, confidential patient records in compliance with HIPAA guidelines, ensuring the integrity and security of sensitive health information.

Qualifications

• Master's degree in Nursing, specializing as a Psychiatric Nurse Practitioner (PMHNP).
• Active state licensure as an Advanced Practice Registered Nurse (APRN), certified in psychiatric care.

• Demonstrated commitment to culturally competent care and an understanding of the varied needs of different communities.

Preferred Experience

• Prior experience in a community health center or similar environment is highly desirable.

Compensation and Benefits
