Work Environment

This position is fully onsite, allowing for direct interaction with clients in their homes. You will work a structured schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events, such as health fairs during weekends, is also anticipated to foster greater engagement within the community. Our home care facility is designed to be a nurturing space that empowers our team to deliver quality care.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for patients, focusing on both physical and mental health in a home care setting.
• Diagnose and manage acute and chronic medical conditions, implementing appropriate preventive care strategies and health promotion practices.
• Work collaboratively with community organizations to design and execute health education initiatives tailored to community needs, such as nutrition workshops and disease prevention seminars.
• Provide culturally sensitive care and advocate for the rights and access of each patient to necessary healthcare services.
• Coordinate referrals and follow-up care to maintain seamless continuity of care for patients, ensuring all health interventions are adequately monitored and adjusted as necessary.
• Maintain accurate and thorough medical records in alignment with regulatory standards and best practices to ensure quality care delivery and compliance.
• Engage in community outreach efforts, awareness campaigns, and health screenings, reinforcing our commitment to public health.
• Stay informed about current community health trends, best practices, and relevant policies to enhance service delivery and improve patient care.

Patient Population / Client Focus

As a Family Nurse Practitioner with Gotham Enterprises Ltd, you will primarily serve diverse populations within the Fort Worth community, including economically disadvantaged groups with limited access to healthcare. Engaging with patients from various cultural backgrounds, you will conduct home visits and evaluate their unique healthcare needs, tailoring care plans that respect and integrate their values and preferences.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license are required.
• A Master's degree in nursing from an accredited program is essential for this role.
• A minimum of 3 years of clinical experience is necessary, with preference given to those with a background in community health or public health settings.
• Demonstrated strong interpersonal and communication skills, with an ability to engage effectively with diverse populations.
• A commitment to social justice, health equity, and the advocacy of marginalized communities is vital.

Preferred Experience

• Experience in community health, home health care, or a similar setting is strongly preferred.

Compensation and Benefits

• We offer a competitive salary range from USD 120,000 to 220,000 yearly, commensurate with experience and qualifications.
