Key Responsibilities

• Conduct thorough assessments of patients in the home care environment, identifying both acute health issues and chronic conditions.
• Develop, implement, and manage evidence-based treatment plans, collaborating closely with physicians and other healthcare providers to optimize patient care.
• Order and interpret diagnostic tests while prescribing appropriate medications and therapies to manage health conditions effectively.
• Educate patients and their families on disease management, medication usage, and post-discharge care to promote adherence and improve health outcomes.
• Participate in interdisciplinary team meetings and rounds to ensure comprehensive care and coordination among service providers.
• Maintain meticulous electronic medical records to document patient interactions, treatment plans, and progress accurately.
• Contribute to quality improvement projects and adhere to evidence-based practice guidelines to elevate the standard of care.

Patient Population / Client Focus

In this role, you will primarily serve home care patients who may have complex health needs, including geriatric patients, individuals with chronic illnesses, and those recovering from acute medical events. Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license granted by the state of Texas.
• Bachelor’s degree in nursing followed by relevant graduate education.
• Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in home care or a similar setting.
• Strong organizational, communication, and interpersonal skills to foster meaningful relationships with patients and families.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 120,000 – 220,000 per year, commensurate with experience and qualifications.
