Work Environment

This position is full-time and requires on-site engagement with patients throughout the Fort Worth area. Key Responsibilities

• Conduct thorough assessments of patients' medical histories, perform physical examinations, and make informed diagnoses regarding acute and chronic conditions.
• Design and implement individualized treatment plans based on the latest evidence-based practices, collaborating closely with physicians and specialists as needed.
• Order, interpret, and communicate results of diagnostic tests, and prescribe medications and therapies while educating patients on their safe use.
• Provide comprehensive education to patients and their families on disease management, lifestyle modifications, and adherence to prescribed therapies for improved health outcomes.
• Engage in regular case discussions and interdisciplinary rounds to share insights and recommendations for complex cases and treatment plans.
• Document all patient interactions, treatment plans, and outcomes accurately in our electronic health record system to ensure seamless continuity of care.
• Contribute to quality improvement projects and initiatives aimed at enhancing patient care and organizational efficiency.
• Stay updated with ongoing changes in healthcare regulations and emerging best practices to continuously enhance clinical skills.

Patient Population / Client Focus

The Nurse Practitioner in this role will focus on a diverse clientele, encompassing individuals with a range of chronic diseases, post-surgical recovery needs, and age-related health challenges. Our aim is to help patients regain and maintain their independence while providing holistic, compassionate care that addresses both physical and emotional health.

Qualifications

• A current and valid Nurse Practitioner certification.
• An active nursing license in the state of Texas.
• A Bachelor's degree in Nursing or an advanced degree preferred.
• A minimum of 2 years of professional nursing experience with a focus on providing quality patient care.
• Exceptional communication skills, with the ability to convey complex medical information to patients and families.
• Strong organizational skills with a capacity to manage multiple patient cases effectively.

Preferred Experience

• Experience working in home care or community health settings is preferred but not mandatory.

Compensation and Benefits
