Key Responsibilities\n

• Perform comprehensive psychosocial assessments to evaluate patients’ emotional, social, and environmental needs, enhancing the understanding of their mental health status.

• Employ evidence-based therapeutic modalities, including cognitive-behavioral therapy and dialectical behavior therapy, tailoring interventions to meet individual patient goals.

• Collaborate with a multidisciplinary team, including psychiatrists, physicians, and nursing staff, to orchestrate coherent and integrated patient care plans.

• Support patients and their families in navigating the complexities of hospice care, including offering education on diagnosis, treatment options, and effective symptom management.

• Facilitate individual, group, and family therapy sessions, ensuring therapeutic interventions are adaptable to a variety of patient needs and family dynamics.

• Contribute to the development of treatment plans that reflect the values and preferences of patients while including clinical insights from the treatment team.

• Advocate for patients' rights and needs within the healthcare system, assisting them in accessing necessary services and resources.

• Lead therapeutic groups, psychoeducation sessions, and discharge planning conferences, focusing on equipping patients and families with the knowledge and skills needed for post-discharge care.

This role primarily focuses on patients receiving hospice care, typically those facing terminal illnesses. You will engage with patients across various demographics, ensuring a sensitive and personalized approach that respects their cultural and individual backgrounds. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
