Key Responsibilities\n

• Collaborate with primary care providers to identify, assess, and treat mental health and psychological issues in patients, thus acting as an integral part of the healthcare team.

• Conduct comprehensive patient intakes that include diagnostic evaluations and functional assessments, followed by providing actionable recommendations for treatment plans.

• Deliver brief, effective interventions tailored for patients in need of mental health services, ensuring timely support and follow-up.

• Monitor patient progress through regular check-ins, adapting treatment approaches as necessary based on patient feedback and clinical assessments.

• Create and implement relapse prevention plans aimed at helping patients maintain stability in their mental health and overall well-being.

• Identify at-risk individuals and develop pragmatic strategies to mitigate the escalation of mental health issues.

• Document patient progress meticulously in treatment charts, maintaining compliance with agency guidelines and best practices for record-keeping.

• Facilitate communication between patients and providers, ensuring that treatment goals are aligned and that necessary services are coordinated effectively.

• Provide educational resources to patients that empower them to participate actively in their treatment journey.

• Offer consultation and feedback to clinic management and team members regarding behavioral health services and program development.

• Adhere to the Code of Ethical Conduct for Clinical Professionals while performing all duties.

Qualifications\n

• A Master’s Degree in Clinical Counseling from an accredited institution is required.

• Current and valid Licensed Professional Counselor (LPC) license to practice in Oregon is mandatory.

• Proficient understanding of developmental psychopathology, DSM-IV diagnoses, and family systems therapy.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
