Key Responsibilities\n

• Conduct comprehensive assessments to identify patient needs and determine appropriate treatment plans.

• Maintain effective communication with supervisors, family members, and the mental health community about clients' progress and care strategies.

• Schedule and coordinate home visits, ensuring adherence to in-home observation standards for effective treatment monitoring.

• Implement and oversee program plans, ensuring they are executed effectively to help clients achieve their therapeutic goals.

• Engage actively in Interdisciplinary Team Meetings, sharing insights and collaborating to enhance treatment methodologies.

• Document monthly and quarterly progress notes, keeping a thorough account of client interactions and outcomes.

• Organize and facilitate training sessions for staff to ensure they are well-equipped to implement the program plans and maintain documentation standards.

• Provide follow-up support through ongoing interviews and assessments to ensure clients' needs are consistently met.

• Serve as a public liaison, working with community resources to develop and enhance support programming.

• Contribute to the development of training materials designed to improve operational effectiveness and care delivery.

Qualifications\n
