Key Responsibilities\n

• Connect patients and families to essential community resources, including support groups, mental health services, and transitional care options, ensuring continuity of support throughout the hospice journey.

• Educate patients and their families about available community resources and services, facilitating understanding and access to post-rehabilitation care and support networks.

• Assist in discharge planning and facilitate smooth transitions for patients moving from hospice care to home or other appropriate care settings, advocating for their needs throughout the process.

Qualifications\n

Compensation and Benefits\n
