Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to the individual needs of patients, considering their mental health, substance abuse, and medical care requirements.

• Provide age and culturally sensitive counseling services to individuals and families facing serious illnesses, including crisis intervention and ongoing support through treatment planning.

• Collaborate closely with interdisciplinary teams to escalate patient needs, ensuring timely interventions from medical providers when necessary.

• Document and maintain accurate electronic health records, including mental health evaluations, progress notes, and suicide risk assessments, adhering to industry standards and regulations.

• Engage with community resources and referral networks to assist patients and families in accessing additional support such as medication assistance programs and garden to plate horticulture therapy.

• Participate in regular quality improvement initiatives, contributing insights and data collection efforts to enhance our integrated care processes.

Patient Population / Client Focus\n

Our patient population consists primarily of individuals at the end stages of life, typically within a hospice center environment. You will work with diverse clients confronting terminal illnesses, which requires specialized knowledge of grief counseling and advanced communication skills to provide the necessary support to both patients and their families.

Qualifications\n

• A valid and relevant California state license as a Licensed Clinical Social Worker (LCSW) is required.
\n
• A Master’s degree in Social Work, coupled with a minimum of three (3) years of post-licensure professional experience in a clinical setting.

• Strong understanding of crisis intervention techniques, counseling methodologies, and the ability to make independent clinical judgments in a direct service capacity.

Preferred Experience\n

• Experience in working with hospice or end-of-life care settings is highly desirable.

Compensation and Benefits\n
