PerformAX Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our dynamic team in our onsite rehabilitation facility located in Chantilly, Virginia. Key Responsibilities

• Develop and implement individualized treatment plans that address the unique needs and goals of each patient.
• Conduct comprehensive evaluations to assess patients' functional abilities and limitations, utilizing diagnostic techniques to guide treatment decisions.
• Administer various therapeutic modalities including manual therapies, therapeutic exercises, and assistive device training to facilitate recovery.
• Oversee and mentor Physical Therapy Assistants, ensuring quality care delivery in line with established plans of care.
• Regularly document progress notes, care plans, and discharge summaries that reflect patient conditions and therapy outcomes.
• Communicate effectively with patients, families, and interdisciplinary team members to ensure coordinated and comprehensive patient care.
• Evaluate patients' home environments to provide recommendations that enhance safety and independence.
• Participate in team meetings and contribute to collaborative treatment planning by sharing insights and strategies.
• Maintain up-to-date knowledge of trends in physical therapy practices and regulations, ensuring compliance in all aspects of care.
• Engage in continuous professional development, including attending workshops and pursuing further educational opportunities.

Patient Population / Client Focus

In this role, you will work with a diverse group of patients, including those recovering from surgery, managing chronic conditions, or rehabilitating from injuries. You will utilize personalized strategies to meet the physical rehabilitation needs of clients across various age groups, creating a supportive environment that encourages recovery and growth.

Qualifications

• Must hold a Bachelor's Degree in Physical Therapy or an equivalent degree, with proof of completion from an accredited program.
• A minimum of one year of clinical experience in a physical therapy setting, preferably within outpatient rehabilitation or homecare environments.
• Current, valid licensure as a Physical Therapist in Virginia, and maintenance of CPR certification.
• A strong commitment to patient-centered care accompanied by effective communication skills.
• A collaborative spirit, with a track record of working within interdisciplinary teams.

Preferred Experience

Compensation and Benefits
