Work Environment

Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, identifying limitations and establishing personalized treatment objectives.
• Design and implement individualized therapy plans that incorporate various modalities including therapeutic exercises, manual therapy, and advanced modalities tailored to enhance recovery and mobility.
• Administer hands-on therapeutic interventions while documenting patient progress accurately and adjusting treatment strategies as needed based on feedback and outcomes.
• Collaborate closely with a diverse interdisciplinary team to ensure cohesive care, sharing insights and strategies to meet patients' needs effectively.
• Educate patients and their families on appropriate exercises to perform at home and provide guidance on lifestyle modifications to support continued recovery.
• Maintain compliance with all facility protocols, state regulations, and best practices in physical therapy.

Patient Population / Client Focus

In this role, you will primarily work with elderly residents recovering from surgeries, strokes, and other debilitating conditions that affect mobility and daily living activities. Your expertise will help improve their functional independence, ultimately contributing to a higher quality of life. Building strong relationships with patients and their families is key as you guide them throughout their rehabilitation process.

Qualifications

• Education: A Bachelor's or Master's Degree in Physical Therapy from an accredited program.
• Licensure: A valid and unrestricted Physical Therapy license in the state of New York is mandatory.
• Strong clinical expertise and excellent communication skills that foster effective collaboration with team members and rapport with patients.

Preferred Experience
