Key Responsibilities\n

• Conduct thorough and ongoing assessments of clients' mental health needs to ensure customized treatment plans are developed and maintained.

• Collaborate effectively with a variety of stakeholders including supervisors, direct care staff, families, and external partners, providing regular updates and reports on client progress.
\n
• Prepare and present schedules of client appointments, home visits, and evaluations to support ongoing involvement and progress tracking.
\n
• Coordinate and execute home visits when needed to fulfill the needs of those receiving in-home support, encompassing chart reviews and staff interviews.
\n
• Monitor and ensure that program plans are implemented effectively and adapt them as necessary to meet clients' evolving needs.
\n
• Participate in interdisciplinary team meetings and other required discussions to enhance collaboration and draw on diverse expertise for client benefit.
\n
• Document monthly and quarterly progress notes to encapsulate the developments within specified timeframes and support continuity of care.
\n
• Organize and oversee training sessions for team members to ensure compliance and competency in program plan implementations and to endorse best practices in documentation.
\n
• Engage in continuous support for clients through follow-up interviews, home visits, and relevant observations to monitor their progress and adapt care plans accordingly.

• Maintain organized and accessible records that facilitate communication and sharing of client information with overseeing professionals and external agencies.
\n
• Act as a liaison with the public and mental health community, advocating for client needs and developing goals for program enhancement.
\n
• Assist in creating procedural documents and training materials aimed at improving therapeutic programs for individuals.

Patient Population / Client Focus\n

Your role will involve working with a diverse range of clients, including both adults and youth, each presenting unique challenges and backgrounds. You will have the opportunity to specialize in areas such as trauma, substance misuse, or family and couples therapy, allowing you to tailor your approach based on specific client needs and preferences.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Nevada.

• Minimum of two years of professional counseling experience, with a preference for candidates who possess experience in a clinical context.
\n
• Strong foundational understanding of legal and ethical standards in mental health practice.
\n
• Exceptional interpersonal and communication skills, able to establish rapport and trust with a varied clientele.
\n
• Demonstrated ability to work collaboratively with healthcare teams, enhancing overall client care through cooperative efforts.

• Proficiency with electronic medical records (EMR) software and relevant technology used to facilitate telehealth services.
