Key Responsibilities\n

• Conduct comprehensive assessments of client counseling needs, including substance abuse evaluations using DSM criteria to formulate an appropriate treatment plan.

• Design, implement, and monitor individualized treatment plans that encompass clients’ substance abuse counseling needs, ensuring evidence-based practices are upheld.

• Prepare and maintain documentation of all counseling sessions, assessments, referrals, and progress notes in clients' clinical records.
\n
• Coordinate client discharge and continuum of care plans, ensuring clients transition smoothly to the appropriate level of ongoing support.
\n
• Provide crisis intervention strategies and facilitate therapeutic services as needed while working closely with a multi-disciplinary team.

• Develop and coordinate family support services, making sure that all interactions are recorded accurately in the clients’ clinical documents.

• Engage with referral sources, such as legal, educational, and occupational entities, to advocate for client needs.
\n
• Manage a caseload, ensuring timely follow-ups and updates to treatment plans according to best practices in patient care.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele facing various challenges, particularly those with mental health and substance use disorders. This includes engaging with patients who have co-occurring conditions, requiring tailored interventions and a compassionate, holistic approach to treatment. Your focus will be on promoting recovery and rehabilitation through evidence-based practices.

Qualifications\n

• Master's degree in social work from an accredited institution, with current LCSW licensure in Pennsylvania.

• Strong capability in clinical assessment and the creation of individualized treatment plans.

• In-depth understanding of best practices for co-occurring disorders and substance use treatment.

• Exceptional ability to maintain therapeutic boundaries and cultural competency when working with diverse populations.
