Key Responsibilities\n

• Conduct thorough biopsychosocial assessments tailored to each individual’s mental health, substance abuse, and medical needs.

• Document all interactions meticulously using electronic health record (EHR) systems, including mental health evaluations, treatment plans, and progress notes.

• Provide individualized counseling sessions, as well as group therapy sessions, to address the social and emotional needs of patients.

• Perform regular suicide risk assessments and take appropriate actions, escalating patient needs to other healthcare providers when necessary.

• Develop and implement treatment plans in collaboration with patients, fostering a support system that promotes recovery and health.

• Act as a liaison between patients and community resources, facilitating referrals to local organizations and programs, including medication-assisted treatment.

• Work closely with Quality Improvement teams to enhance service delivery and ensure compliance with clinical standards.

Patient Population / Client Focus\n

Your primary focus will be on individuals experiencing substance use challenges and co-occurring mental health disorders. You will engage with patients from various backgrounds and demographics, tailoring your approach to meet diverse cultural, age-related, and personal needs. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited program.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
