Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews and observations to gain insight into the emotional, social, and environmental factors impacting each patient's recovery journey.

• Provide tailored counseling and therapeutic support to patients and their families, addressing specific adjustment issues that arise as they navigate their rehabilitation.

• Collaborate effectively with healthcare providers within the rehabilitation team to develop comprehensive and individualized treatment plans that address patients' psychosocial needs.

• Assist patients in identifying and accessing essential community resources, such as support groups, vocational rehabilitation programs, and housing assistance to facilitate their reintegration into society.

• Actively participate in discharge planning, ensuring a smooth transition for patients moving from rehabilitation into home or other care settings, and remain available for follow-up support.

• Educate patients and families about available post-rehabilitation services and community resources, empowering them to utilize these supports effectively.

Patient Population / Client Focus\n

In your role as an LCSW, you will primarily serve a diverse patient population facing a variety of challenges related to rehabilitation. This may include individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Our focus is on maximizing functional independence, helping each patient to achieve their personal goals for recovery and reintegration into their communities.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

Compensation and Benefits\n
