Overview Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Responsibilities Bring your heart into every visit and join a national network of purpose‑driven clinicians dedicated to improving health outcomes. Help visit millions of people nationwide, connecting them to the right care for their unique needs. Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role. Be flexible to travel locally and within licensed states. Conduct virtual visits as needed. Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, and more. Work Environment As an in‑home clinician, your primary workplace will be the private residence of health plan members. You must be able to safely perform duties in homes that may contain household pets, secondhand smoke, or other allergens. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Able to obtain and maintain multi‑state licenses in coverage area(s) as required. Travel Requirements Travel up to 100% of the time across a defined coverage area. This may require overnight hotel stays, car rentals, and/or flights.
