Sign-on Bonus: In-Home VA Traveler, Nurse Practitioner or PA

Position Summary Signify Health, part of CVS Health®, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves traveling up to 100% of the time across a defined coverage area, including overnight stays, car rentals, and/or flights as required. Holding multiple state licenses is required, as dictated by business needs. Key Responsibilities Perform in‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical assessment, and requested diagnostic tests. Use an iPad and intuitive clinical workflow to document and record findings. Do not prescribe medication, order lab tests, or alter current treatment regimens. Build trusted relationships with patients, providing connections to appropriate care based on their unique needs. Conduct virtual visits as needed. Provide Diagnostic & Preventive Services such as spirometry, diabetic eye exams, bone density screening, and other permitted tests. Travel flexibly to meet local and inter‑state coverage requirements. Ensure safe performance of duties in diverse home environments that may include pets, secondhand smoke, or allergens. Maintain all necessary state licenses and certifications throughout the coverage area. Required Qualifications Active, unrestricted license(s) in all coverage area states. Board certification in the appropriate specialty. Ability to obtain and maintain multi‑state licenses as required. Capability to cover all counties within each state and to flex to additional areas per business needs. Preferred Qualifications Demonstrated flexibility and willingness to travel extensively across coverage regions. Experience performing in‑home or telehealth clinical evaluations. Strong communication skills and a compassionate bedside manner. Employment Details Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: $87,035.00 – $187,460.00 (base salary, varying by experience, education, geography, and other factors) Benefits: Health, dental, and vision insurance; paid time off; annual CEU allowance; 401(k); malpractice coverage; supplies and other perks.
