Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments for clients, meticulously evaluating their mental health conditions and substance use concerns to formulate effective treatment plans.

• Provide individualized, group, and family counseling sessions that are sensitive to the unique needs, backgrounds, and situations of clients facing end-of-life issues.

• Facilitate connections to local community resources, including support groups, rehabilitation programs, and medication management assistance, to complement therapeutic interventions.

• Collaborate with improvement teams to continuously enhance care processes, sharing insights and contributing to the development of best practices in hospice care services.

• Participate in case conferences and team meetings, integrating feedback to adapt care strategies and improve patient outcomes.

Patient Population / Client Focus\n

The primary focus of this role involves working with patients and families associated with hospice care, a sensitive area that requires a deep understanding of grief, loss, and emotional adjustment. You will provide support during some of the most challenging periods in clients' lives, employing empathy and active listening to foster a trusting therapeutic environment. Your expertise will not only aid in individual acceptance and coping mechanisms but also assist families in communicating and processing their experiences.

Qualifications\n

• Master’s degree in Social Work from an accredited program.

• Current Licensed Clinical Social Worker (LCSW) license in the state of Oregon.

Compensation and Benefits\n
