Nurse Navigator - Full Time - Day

About This Role The Nurse Navigator at Hackensack Meridian Health collaborates with surgeons and the healthcare team to navigate patient care, focusing on ongoing assessments of patient and family needs and connecting them to resources. This role is ideal for experienced RNs who excel in patient advocacy and coordination. Highlights Location: Neptune, NJ Schedule: Full-time, Day shift What You'll Do Coordinate and navigate outpatient care for patients. Identify new patients as they are diagnosed and offer navigation services. Provide initial detailed patient and family needs assessment and periodically update it. Develop disease site expertise. Provide patient and family education. Connect patients and families with available resources in the health system and community. Maintain a database of available community resources. Facilitate access to services. Track the flow of patients through diagnostic and treatment services. Document assessment and follow-up notes in the patients' medical record. Requirements Education: Graduate of licensed school of nursing; RN required. Experience: At least 5 years of clinical experience in a relevant clinical specialty. Licenses & Certifications: NJ State Professional Registered Nurse License required. AHA Basic Health Care Life Support HCP Certification required. National Certification preferred.
