Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to determine the developmental needs of children.

• Participate in the planning and implementation of Individualized Family Service Plans (IFSPs) tailored to meet the unique circumstances of each child and family.

• Monitor and evaluate outcomes collaboratively with team members to ensure families receive the support they need.
\n
• Provide thorough support for families, equipping them with knowledge and practical skills to enhance their child's development.
\n
• Adhere to all federal and state regulations governing social work practice.
\n
• Facilitate the development of a supportive, trusting rapport with children, families, and colleagues, fostering a healthy learning environment.

• Utilize coaching models and natural environment teaching strategies to support family engagement.
\n
• Implement IFSPs for enrolled families and make recommendations regarding service frequency based on child development status and family feedback.

Patient Population / Client Focus\n

This role primarily focuses on children from birth to three years old who may have disabilities or delays in their developmental milestones. You'll be engaged with their families in crafting actionable plans that address physical, cognitive, social/emotional, and communication skills. Your work will not only influence the children's immediate developmental needs but will also lay the groundwork for their future social and learning experiences.

Qualifications\n

• A Master’s degree or Doctorate in Social Work, with LCSW licensure from the Department of Public Health.

• A valid driver's license in order to travel for assessments, if necessary.

• Excellent organizational and time management skills to effectively juggle multiple responsibilities.

Preferred Experience\n

Compensation and Benefits\n
