Key Responsibilities\n

• Provide clinical consultation and support to clinical staff on mental health assessments, specifically addressing issues affecting various patient populations such as geriatrics and individuals dealing with substance use disorders.

• Interpret and effectively communicate essential patient information, including social histories, diagnosis, and treatment plans, to staff and healthcare providers.

• Coordinate individualized patient care planning by assessing home situations and making actionable recommendations that enhance patient welfare.

• Plan and review treatment strategies, critically evaluating provider case notes and ensuring proper documentation of client progress.

• Perform various other duties as assigned to support the overall functionality of the team and enhance client care.

Patient Population / Client Focus\n

You will primarily work with a diverse clientele, including children, adolescents, adults, and seniors facing mental health challenges, substance use issues, or trauma-related behavioral problems. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Current independent licensure such as LMHC, LCSW, or LMFT is essential.

Compensation and Benefits\n
