Key Responsibilities

• Conduct comprehensive evaluations and assessments for patients and their families, using a holistic approach to understand their needs and concerns effectively.

• Actively monitor and track patient outcomes as an integral member of the interdisciplinary team, contributing your expertise to improve overall care quality.

• Provide emotional and practical support for families, equipping them with information and skills necessary for enhancing the care of their loved ones.

Patient Population / Client Focus

This position serves a diverse patient population within our hospice center, focusing primarily on individuals with terminal illnesses and their families. You will work to provide emotional support, navigate complex feelings surrounding end-of-life care, and facilitate practical coping strategies that respect each family's unique circumstances and preferences.

Qualifications

• A minimum of 2 years of professional experience as a social worker, particularly with a focus on providing support within the early intervention and hospice fields.

• An earned Master’s degree in Social Work from an accredited institution, confirming your competence and knowledge in clinical practices.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• Exceptional organizational and time management skills, essential for balancing multiple cases and ensuring timely delivery of services.

Compensation and Benefits

The salary for this position ranges from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
