The Behavioral Wellness Oasis Association is seeking a dedicated and fully licensed Licensed Clinical Social Worker (LCSW) to join our remote mental health clinic, specifically focused on hospice care. Key Responsibilities

As a Licensed Clinical Social Worker, your primary responsibility will be to deliver high-quality psychotherapy to individuals experiencing a variety of mental health issues, particularly in the context of hospice care. Key duties include:

• Conducting comprehensive clinical assessments to understand patient needs, developing individualized treatment plans that align with best practices in grief counseling and crisis intervention.
• Implementing various therapeutic interventions including but not limited to cognitive-behavioral therapy (CBT) and acceptance and commitment therapy (ACT), tailored to the unique needs of hospice patients and their families.
• Maintaining meticulous clinical documentation in accordance with HIPAA standards, ensuring that patient records are up-to-date and easily accessible for other team members.
• Collaborating with an interdisciplinary team, including physicians and nurses, to create a holistic care approach that addresses both the physical and psychological needs of patients.
• Participating in case consultations and peer supervision to enhance therapeutic strategies and foster team cohesion.
• Educating patients and families on mental health issues related to end-of-life care, empowering them with resources and support networks.
• Engaging in continuous professional development by staying abreast of the latest research and treatment methodologies in the mental health field.

Patient Population / Client Focus

Your patient population will consist mainly of individuals and families navigating terminal illnesses. You will provide emotional support and therapy to patients facing end-of-life transitions, as well as their loved ones who may be grappling with grief, anxiety, and other mental health challenges. Your empathetic approach will play a key role in enhancing the quality of care delivered in hospice settings.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Connecticut, indicating your credentials to practice as a Licensed Clinical Social Worker.
• At least 2 years of clinical experience, particularly in providing psychotherapy, is essential for this role.
• Deep understanding and proficiency in solution-focused psychotherapy techniques and evidence-based practices.
• Exceptional skills in communication, problem-solving, and collaboration.
• Ability to work independently in a remote setting while being an integral part of a team.

Preferred Experience

• Previous experience working within hospice, palliative care, or related settings preferred.
