Remote LTSS Care Manager & Care Coordination
Molina Healthcare Inc
Molina Healthcare Inc is seeking a Care Manager for LTSS to support care coordination across Wayne County, MI, with remote options. You will conduct comprehensive assessments, develop and monitor care plans, and collaborate with physicians and the care team to meet member outcomes.
