This position is primarily remote, allowing you the flexibility to work from home while maintaining close communication with the interdisciplinary rehabilitation team. Regular business hours are expected, with occasional evening and weekend shifts based on patient needs and assigned caseloads.

Key Responsibilities\n

• Conduct thorough psychosocial assessments through patient interviews, observational methods, and medical record reviews to evaluate emotional, social, and environmental factors affecting each patient's recovery journey.

• Provide empathetic counseling and support to patients and their families to address the adjustment issues associated with severe illness and end-of-life scenarios.

• Work closely with the interdisciplinary rehabilitation team to develop individualized treatment plans that incorporate psychosocial needs alongside physical rehabilitation goals.

• Facilitate access for patients to essential community resources, such as support groups, vocational rehabilitation services, and housing assistance, ensuring a well-rounded approach to their recovery.

• Educate patients and families about available community resources and post-rehabilitation services to promote successful transitions back to community living.

• Participate in discharge planning to ensure smooth transitions to home or other care settings, addressing potential psychosocial barriers that could impede the patient’s recovery process.

Patient Population / Client Focus\n

You will work with a diverse patient population, including individuals facing terminal illnesses, those with chronic pain, and patients requiring continuous rehabilitation support. Your role is vital in addressing the emotional and social complexities these patients face, fostering a sense of dignity and hope while promoting their independence and quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
