Key Responsibilities\n

• Provide direct clinical services, including assessment, diagnosis, treatment planning, and interventions for clients experiencing severe emotional disorders and mental illnesses within our hospice setting.

• Conduct thorough intake assessments and regular follow-ups to evaluate client progress, adjusting treatment plans as needed to optimize therapeutic outcomes.

• Facilitate individual and group therapy sessions using evidence-based practices and trauma-informed care strategies.

• Engage in multidisciplinary team meetings to collaborate on patient care strategies, sharing insights and information to enhance the collective support offered to clients.

• Act as a resource for clients and families, providing education on coping strategies and available community resources to promote empowerment and informed decision-making.

• Document all clinical interactions promptly and accurately, ensuring compliance with healthcare regulations and ethical standards.

Qualifications\n
