Key Responsibilities\n

• Utilize your educational background, training, and practical experience to conduct comprehensive psychosocial assessments and develop accurate diagnoses.

• Implement evidence-based behavioral interventions, including but not limited to solution-focused therapy, motivational interviewing, and behavioral activation, to foster positive treatment outcomes.

• Engage in teamwork with other healthcare providers, including clinicians and psychologists within our organization, as well as external primary care providers to endorse a holistic approach to patient care.

• Systematically monitor patient progress toward treatment objectives, employing both in-person and virtual check-ins to adjust and enhance care plans as necessary.

• Assist in the management of psychotropic medications prescribed by medical providers, focusing on adherence, side effects, and overall treatment effectiveness.

You will primarily work with patients in a hospice center, providing them and their families with necessary support during a pivotal and often challenging time.
